Please arrive a few minutes early for your appointment. Bring all medications with you. Every effort is made to keep scheduled appointment times. Occasionally a hospital emergency might cause a delay. If this happens, we will inform you and offer you an alternate appointment.
Let us know at the time of your appointment if any of your personal information, such as address or phone number has changed. Each time you come we will request a copy of your insurance information. The insurance information is attached to your charge ticket to assist us in processing your office visit. Any co-payments or co-insurance will be due from you at the time of the service.
New Patient Appointments
Cardiovascular Consultants schedules new patients by physician referral. Your family physician will contact our office to schedule an appointment for you. Demographic information is gathered from your family physician and an appointment is scheduled. There are times when the family physician refers a patient to us and has the patient call to confirm an appointment time.
New patients will receive a patient questionnaire and registration in the mail, which you need to complete and bring to your first appointment. The questionnaire requests your demographic and insurance information as well as medical history. Please bring the following to your first appointment:
Completed questionnaire
Current insurance cards
Medications you are taking
Appointments Reminders Patients will receive an automated reminder through the phone. This message will remind you of the details of your appointment, time, day, and provide special instructions. The system gives you the option to confirm your appointment or
respond that you will not be keeping the appointment and will call to reschedule. Please select the appropriate response.
Missed Appointments
Patients that do not show up for an appointment consume the resource of the physician’s wait to receive an appointment. Please give respect to your physician and be considerate of your fellow patients by notifying the office at least 24 hours in advance if you will not be able to keep your appointment. Failure to keep an appointment may result in a service charge.
Walk-In Patients
It is important that you have an appointment to see your physician. If you are experiencing acute symptoms, call 911 or proceed to the nearest Emergency Room.

Cardiovascular Consultants is a Provider for the following

If you have a question about whether Cardiovascular Consultants participates in an insurance plan, please ask at the time you make your appointment. It is your responsibility to provide us with accurate timely insurance information whether you are seen in the office or hospital setting.
All insurance claims are filed as a courtesy. Your account is your responsibility. It is in your best interest to know your insurance benefit plan. Payment of co-pays and co-insurance amounts is expected at the time of service.
We accept several methods of payment, cash, personal checks, money orders, Visa and MasterCard. Short-term payment plans are available in special circumstances and require an established payment arrangement.
Our Patient Accounts personnel is available to discuss your insurance and payments needs. If you have no insurance, you are responsible for payment at the time of service unless prior arrangements have been made. Representatives are available to assist you between the hours of 8:30 am and 4:30 pm, Monday through Friday by calling (573) 651-0439.
Administrative Services and Medical Records
Requests for completion of Attending Physician Statements, Disability, and Specified Disease forms require a pre-payment of $35.00. The forms can be brought or mailed to the office. Please allow 7 – 10 business days for completion of the form.
CVC complies with Missouri Section 191.227, to set the handling and per page fees for medical records
at $19.31 for handling and $.46 per page. Pre-payment is required.
An authorization for the release of medical records is required to receive and send information.
Prescription Refills
Please do not wait till your medication runs out to request a prescription. We recommend that you call your pharmacist for refills a week in advance. Your pharmacist will coordinate your refill request with our office. Please call the office for written prescriptions. Allow 48 hours for refills or written prescriptions to be completed.
Lab and Test Results
Though most diagnostic imaging centers have testing results available within a few days, your CVC physician must review the results before the information may be released to you. CVC physicians are scheduled in the office two days a week. We will have the results available to you within 7 to 10 business days from when the test was performed. In most cases, the physician will request a nurse or medical assistant to give you the results.
Health Insurance Portability and Accountability Act (HIPAA)
Cardiovascular Consultants complies with the Federal Government’s mandated HIPAA requirements for patient confidentiality and privacy of healthcare information. Each patient is to review and acknowledge receipt of our HIPAA Privacy Practice. Patients may identify a representative on the HIPAA Acknowledgement that will allow Cardiovascular Consultants to discuss medical information with that individual.
